This form, when completed and signed by the Parent or Legal Guardian and received by the Legacy Baseball and Softball Club, is the official uniform request. Each player must submit a uniform request form prior to the ordering thereof. This form will be utilized to order your athletes uniform. If a number request is a duplicate, numbers will abe allocated based on returning players, new players number will be allocated by date of birth. All softball pant styles will be knickers. A uniform sizing kit will be provided and each player will be provided the opportunity to try on uniform for proper sizing. Upon uniform order, any and all discrepencies in uniform fitment that results in a re-order will be done so at the expense of the player's Parent or Legal Guardian. Payment for re-order must be submitted prior to ordering thereof to include shipping and handling. Any and all discrepencies in uniform such as name spelling, etc. that is not of any fault of the players Parent or Legal Guardian will be re-ordered at the expense of the Club.
